Cannot find the form you need? The Law Library has books and online resources that you can use to find sample language to use in a form you create. When creating your own form, you will need to make sure it follows the Statutes and the Rules of Court, otherwise the court may reject the forms. Washoe County Law Library provides free access to Westlaw. Westlaw is an online legal research service providing quick, easy access to West's vast collection of statutes, case law materials, treatises, forms, and other legal resources, as well as primary law for the U.S. and all 50 states.
The Second Judicial District Court does not currently have forms for Adoption. If you need these forms, please visit www.selfhelp.nvcourts.gov (external link, open in new tab). Once you are on the website click, ALL OTHER LOCATIONS. This will take you to the Nevada Supreme Court approved forms.
If you would like to appeal a District Court decision, appellate forms can be found on the Supreme Court of Nevada's website.

A Deed Upon Death allows an owner of real property to create a deed which conveys his or her interest in the property to a named beneficiary or beneficiaries upon the owner’s death. A deed upon death may be revoked at any time by the owner. This form is filed with the County Recorder, not the Court. For further information on Deeds upon Death please refer to Nevada Revised Statutes 111.655 to 111.699.

If you do not have an existing title, or other adequate proof of ownership of a vehicle, and you are unable to contact the previous owner for the necessary documentation, please go to the DMV website (external link, open in new tab) to apply for a bonded vehicle title.
